"The steps have 4 square holes to accommodate receiving of the carriage bolts you slide through the step and ladder flange. Fours steps and four bolts each = 16 bolts and lock nuts. The other broken bolt occurred before the lock nut was even all the way up on the bolts and not even tight yet. And, the carriage bolts are a bit small for the holes cut in the steps, causing the bolts to "turn. easy in the soft aluminum steps. The usage instructions advise ladder climbers to be sure to put there feet "in the middle of the step" vs to the left or right side of the step. I cannot imagine the number of people who will be using the ladder and I am sure many times no one will be their to monitor them and tell them they have to step in the middle of the step."

Ideally, if the anchor gets stuck, you can pull on the rope hard enough causing the zip tie to break and changing the angle your pulling at and hopefully dislodging the anchor."
